Verify Your Service

Annual Verification

In order to process your Annual Verification you will need the following:

• Last 4 digits of your Social Security Number
• Your Date of Birth
• Your Enrollment ID or your Zipcode

You may also call us directly at 855-711-2222 and we can process your annual verification over the phone. You will be required to supply us with the information mentioned above.

We are glad we are able to provide you with the service you need to reach the people you care about most. To maintain your service, you must verify your eligibility with Care Wireless every year, per Federal and State regulations. You must certify that you are still eligible to receive Lifeline Service and are still receiving benefits from at least one of the following government assistance programs:

• Food Stamps
• Medicaid
• Federal Public Housing Assistance . Section 8
• National School Free Lunch Program
• Bureau of Indian Affairs Programs
• Supplemental Social Security . SSI
• Temporary Assistance to Needy Families . TANF
• Low Income Home Energy Assistance Program . LIHEAP

Or you must certify that you are still eligible to receive Lifeline benefits based on your income being at or below 135% of the Federal Poverty Guidelines (FPG). Please use the following chart to determine if you are still eligible based on your annual household income

And, no one who currently resides in your household currently receives Lifeline Assistance. If you or someone else in the household is currently receiving Lifeline Assistance you must cancel the service before applying for Care Wireless Lifeline Service.

You must have a valid United States Postal Address. In order for us to ship you your FREE Care Wireless phone, you must live at a residence that can receive mail from the US Post Office. P.O. Boxes cannot be accepted.
